On your first visit we will gather a lot of important background information about you and we will give you time to get to know your doctor. You can use much of the information on our web-site to prepare for your first visit by reading about our staff, reading information about the various treatments we offer, and by downloading forms you can fill out prior to arriving at our office. Taking these simple steps will help alleviate any anxiety you may be experiencing and will provide us with the information we need to give you the best care possible. We are looking forward to meeting you! Patient Forms and Policies•

What to Expect on your First VisitYour initial appointment will consist of a consultation explaining your diagnosis and treatment options. Occasionally, treatment can be done the same day as the consultation. However, a complex medical history or treatment plan will require an evaluation and a second appointment to provide treatment on another day. • Your referral slip and any X-rays if applicable.
